Please download the documents entitled US Club Player Registration and Medical Release Form, NJ Youth Soccer Player Membership Form, and NJ Youth Soccer Medical Release Form, to your computer. The three forms are in WORD format. Please complete and sign all three forms. It will also be necessary to supply a copy of the player’s birth certificate at the scheduled player carding session.
